Support coordination is a specific support ​funded under the National Disability ​Insurance Scheme (NDIS) that aims to ​help you understand your plan, build your ​capacity and pursue your goals.

  • Hospital Discharge Planning (Prepare your ​discharge with the Hospital Liaison Officer)
  • Post-discharge support implementation ​(arranging therapy services, income support, ​urgent assistive technology and home ​modification assessment) to make sure you ​are safe to return to your home.
  • Assist you in planning interstate or ​international travel (arranging support, ​medical clearance and requesting reasonable ​adjustment from the airlines).
  • Connect you with Mainstream Services for ​services that are relevant to you (justice, ​financial, housing, mental health, AOD, and ​other things)
